Why 2nd Home?
2nd Home Adult Medical Day Care is an alternative program that enables participants to live at home in the evenings and on the weekends while receiving individualized care for their medical, nursing, social, rehabilitative and personal needs at our centers, Monday through Friday.
2nd Home Adult Medical Day Care has been providing care, support, companionship and counseling for elderly and handicapped adults who require supervision during daytime hours since 1998. Why 2nd Home Adult Medical Day Care? 2nd Home Adult Medical Day Care Centers provide a program of social, recreational health activities and services in a group setting that:
- Offers participants the opportunity to socialize, enjoy peer support, and receive health and social services in a stimulating and supportive environment that promotes better physical and mental health
- Provides assistance to families and other care givers who have the responsibility for an older adult who cannot be left alone during the day and yet does not require 24-hour nursing care in an institution

Am I eligible?
2nd Home Medical Day Care participants include those who are:
- Physically Impaired
- Socially Isolated
- In need of assistance with personal care
- Mentally Confused
- Limited in their ability to function independently in the community
Eligibility is for anyone who is over 18 years old, who requires on-going skilled services of a nurse, social worker, physician assistant or dietician. More specifically, 2nd Home is ideal for those who have a medical diagnosis that causes impairement, but do not require 24 hour care.
FAQ
How does it work?
How many days a week can I come to 2nd Home? Upon admission you will be evaluated by a registered nurse. The registered nurse in conjunction with the administrator will determine how many days per week you will attend program.
How long do I have to stay in program? The program is for a minimum of five hours per day, not inclusive of transportation. How will I get to 2nd Home? 2nd Home will provide you with transportation to and from the facility. In addition, we will transport you to your physician appointments.
How can I pay for Adult Medical Day Care? Services provided by 2nd Home Adult Medical Day Care are covered 100% by Medicaid, but only to qualified members. Some private insurance companies and community programs cover adult day care services also.
